• Your Carphone Warehouse account number
• A cheque for £10 (admin charge) made payable to The Carphone Warehouse
• A copy of one of these proofs of identity
  • Birth certificate
    
  • Passport
  • Driver's License
  • Credit or debit card statement
  • Utility bill showing the same name and address as on your TalkTalk or Carphone Warehouse account (but not a TalkTalk or Carphone Warehouse bill)
  • Child benefit book
  • Pension book

We recommend that the request is sent by Recorded Delivery, but this is not compulsory. We can accept postal orders but we do not take online payments.

What happens next
