Statement of demands and needs There are two insurance policies available to mobile phone customers. - Geek Squad Mobile - for pay as you go mobile phone customers who want their new phone insured against theft and damage.
- Geek Squad Mobile Max - for pay monthly mobile phone customers looking to insure their new phone against theft, loss and damage.

Name of Insurer. Aviva Insurance UK Limited (Aviva) underwrites this policy.
Two agreements. When you take out your Geek Squad insurance, you enter into two contracts; one with Aviva for your insurance policy, and one with The Carphone Warehouse for Geek Squad technology support and ACP (if selected). If one of these agreements is terminated, the other will automatically be terminated.
Eligibility for this policy. Anyone 16 or over, provided you've bought a mobile phone from The Carphone Warehouse within the last 30 days.
Period of Cover. Cover will start from the start date shown on the certificate.
> Geek Squad Mobile Max - we will continue to take payments every month for this policy unless it is cancelled.
> Geek Squad Mobile - we will write to you at least 21 days before the expiry date to offer you a renewal. The policy will only cover the mobile phone listed on the original certificate of insurance or a replacement mobile phone provided by The Carphone Warehouse as a result of an insurance claim or a warranty claim.
What is not covered:
> For each claim that you make you must pay the excess shown on your insurance certificate. > Claims for loss are not covered by Geek Squad Mobile. > Claims for theft, loss or malicious damage unless you:
1. For theft or loss, notify your network provider within 24 hours of discovering the incident and have your mobile service line barred; 2. In all cases, tell the police within 48 hours ofdiscovering the incident and get a crime/loss reference number. > Claims if your mobile is left unattended in a public place, or in a vehicle, unless the vehicleis securely locked and the mobile is placed out of view. > Claims if your mobile is dependent on a SIMcard or network service provider to work and it does not contain a SIM card or is not connected to a network service provider at the time of the incident. > Claims outside the UK if you are abroad for a period of more than 60 consecutive days. Your mobile phone accessories unless they were purchased from The Carphone warehouse and were lost, stolen or damaged at the same time as your mobile.
Cancelling your policy. You can cancel your policy at any time.
A full refund will be made if you cancel within 14 days of receiving your Geek Squad Policy documents. If you make a claim then cancel, we may ask you to pay back money or return any replacement mobile.
If cancelling after the first 14 days and part way through a month, no partial premiums will be refunded. With annual insurance policies after the first 14 days, you will be entitled to a refund of premium, subject to a deduction for the time you have been insured. |

Making a complaint. We hope you are very happy with our service. However, If you are unhappy, we would like to hear from you.
Always include your name and address with the Policy number shown on the Certificate. You can obtain a copy of our internal process for handling complaints using the contact details outlined below. If you are dissatisfied with our final decision, you can within 6 months refer the matter to the Financial Ombudsman Service (FOS) in writing at South Quay Plaza, 183 Marsh Wall, London E14 9SR.
